Hemodialysis orders from your attending physician
In-house nephrologist
Hepatitis B & C Screening (HbsAg / Anti HBs / Anti-HCV / Anti-HBc total) within the last 3 months
Latest laboratory results (CBC, Blood chemistries, etc.)
If transferring from other Hemodialysis Center:
Clinical Abstract/ or Medical Record
Hemodialysis Treatment Sheet (last 3 treatments)

We have currently opened ourselves to serve as a COVID-19 dialysis center. Please inquire with our administrator (details in Contact Us) with regards to the eligibility of admission within our center. Moreover, since we only use a few machines for patients with COVID-19, please expect that there will be a limited number of available slots for treatment sessions.
